System Entry Test Blueprint

System Entry Test Outline

Our goal with system entry is to assure your competency as a paramedic. There is a written exam and oral exam that is based on the EMS Region 8 Standard Operating Procedures (SOPs). Based upon the results of these evaluations, the examiner may request that you take an additional practical skills review. A recommended study reference is Brady’s Paramedic Emergency Care.

Failure to notify our office that you are unable to keep your appointment will be recorded as a system entry failure.

YOU MUST PROVIDE THE FOLLOWING DOCUMENTATION PRIOR TO TAKING THE EXAMS:

______ Letter of good standing from your current EMS system

(Your current system may fax this form directly to us at 708-327-2548)

______ Photocopy of your current Illinois EMT-P License

______ Photocopy of your current CPR Healthcare Provider level card

______ Photocopy of your current Illinois Driver’s License

______ Completed Personal Data Form (available on System Documents page)

______ Completed Release of Information Form

______ System entry application fee of $25.00

Loyola EMS only accepts cash, credit cards or money orders for system entry.

Should your first attempt at system entry be unsuccessful, you may schedule a re-test. A second unsuccessful system entry session will result in a mandatory waiting period of six months.

We hope to assure that your entry into the Loyola EMS System will be completed in a smooth and effective manner. Please call us at 708-327-2547 with any questions or concerns.
